Surgical approaches to the deep midface and nasopharynx frequently involve osteotomies or partial or complete maxille~tomies.~-~ Osteotomies at the Le Fort I, 11, and I11 levels are commonly used to correct dentofacial deformities, and the vascular anatomy and surgical techniques are well When these osteotomies are used to facilitate tumor extirpation, the vascular anatomy takes on even more importance. In this article we would like to discuss the vascular anatomy of the maxilla, the surgical technique of the Le Fort I osteotomy, and consider various uses and vascular limitations of this approach.
