For example, the range of circulating level of human growth hormone in healthy children is between 1 and 10 ng/ml (11). Phenotypic correction of hemophilia could be obtained with a plasma concentration of Factor VIII or Factor IX around 10% of the normal level (the normal level is respectively 100 ng/ml and 2 g/ml) (12, 13). The gene therapy approach presents many advantages over classical therapy which consists of daily injections of recombinant protein. Intramuscular transfer of a gene coding for a secreted protein leads to lower but prolonged plasma levels of the protein compared to the high concentration peak induced by the injection of the recombinant protein, followed by rapid blood clearance.

In summary, secreted alkaline phosphatase is a secreted reporter protein that allows a very sensitive and convenient detection in vivo. This is an efficient tool to study new vector constructs to improve their efficiency, to study gene regulation, or to study the efficiency of different methods of gene transfer. This new tool for rapidly assessing the in vivo secretion of transgene product could be most valuable for the development of gene therapy applications that involve circulating or locally secreted therapeutic proteins.
