Oigtbmd is a stone, lyopNllzed powder of anhgen bmdmg fragments (Fab) denved from specific anhdtgox-[n antibodies raised In sheep CONTRAINDICATIONS: None known WARNINGS: Sutctdal Ingestion often involves mere than one drug and thts posstblhty must be constdered Anaphylachc, hypersensdtvity, or febrile reachons are posstble Patients with known alJergtes (especially allergies to anhblollcs, papain, ehymopapaln, or other papaya extracts), or who have previously received antlbodtes or Feb fragments ratsed in sheep may be at nsk of having an Immunogenle response Skm testmg with Dtgtbmd ts not routinely required before treatment of life-threatening dtgttahs intoxication but may be appropnate for htgh nsk patents, especially those described above If skin teshng causes a systemic reaction, a tourntquet should be apphed above the site of testing and measures to treat anaphylaxis mttiated If further administration of DJgtblnd Is absolutely essenhar, the pahent should be pretreated with corttcosterolds and dtphenhydrarnme and preparations should be made to treat anaphylaxts Discontinue drug infusion tf an anaphylactotd reaction occurs, and inttiate appropnate therapy The need for epmephnne should be balanced agamst tts potentla~ nsk in the setting of d~glta]ts toxtctty PRECAUTIONS: Patients should be closely monrtored dunng and after Dtgtbind adrnmlsfration, including temperature, blood pressure, ECG, and serum potassium concentrahons Masswe digitahs intoxication can cause hyperkalemJa, adrnlntstrahoe of potassium supplements/n thte setting may be hazardous After treatment with Dtgtbmd, the serum potasstum concentration may drop rapidly and must be monttored frequently, especially durmg the first few hours after treatment wtth Digtbmd Pahents with severe renal fadure should be monitored for a prolonged penod following Dig~blnd treatment for possible recurrence of d~gltahs toxtctty Cardiac function may detenurate from withdrawal of the inotroplc actfon of digoxm When necessary, addlhonal support can be provtded by use 01 mtravenous motropes or vasodllators Catecholamlnes must be used carefully to avofd aggravahng digitalis toxic rhythm disturbances Other types of digitahs glycosides should not be used m this settmg Postpone redigltahzation if possible, until the Fab fragments have been ehmTnated from the body Ehrnmahon may requtre several days and, in pattents with tmpalred renal funchen, a week or longer Laboratory Tests: If possible, serum dlglta]ts concentrahun should be obtained before adrnmtstermg Dlgtbmd Dlglblnd mterferes wdh the standard serum dlgoxln concentration assays The total serum dtgoxln concentrahon may nse precipttously after Digtblnd adrnmlstratmn This Increase represents dlgOXin bound to Fab and is, therefore, inactive The standard serum dtgoxtn concentrahon measurement thus can be clinically misleading until the Feb fragment Is ehrnmated Serum potassium concentrahon should be followed carefully Carcinogenesis: No long-term ammal studies have been performed to evaluate caremogentc potenttal Pregnancy: Pregnancy Category C Antmal reproduchon studtes have not been conducted with DtgJbmd Digibmd should only be given to a pregnant woman If ctearly needed Nursing Mothers: It ts not known tf Digtbind is excreted m human rntlk Cautton should be used m adrnmlstermg Digiblnd to nursmg mothers Pedtatrio Use: Dig@nd has been success{ully used m Infants wtth no apparent adverse sequelae Use, however, should always be based on a beneht to r~sk assessment ADVERSE REACTIONS AIlergtc reactions to Diglbind have been reported rarely (See WARNINGS,) Hypokalernia may occur from re-achvation of (sodtum, potasstum) ATPase Patients wtth atnat flbrJllattun may develop a rapid ventncular response from withdrawal of the effects of dtgttal~s on the atnoventrleular node Low cardlac o~Ltput states and congeshve heart failure may be exacerbated by withdrawal of the inotroptc effects of dtgttahs
