Syphilitic infection of the central nervous system can occur early in the disease process, and signs can be varied and atypical. 1,2 Here we present a unique case of neurosyphilis presenting with myoclonus, cerebellar ataxia, and speech disturbance in the setting of executive dysfunction and anosognosia.
