but in his cases the onset was some years after the operation, and the site of the obstruction was at the gastrojejunal anastomosis.
SUMMARY
Two cases are recorded of sudden early postoperative duodenal distension following Polya gastrectomy. T h e distension was apparently produced by a 'physiological' obstruction at the duodenojejunal flexure. The distension of the duodenum was so great that the duodenal stump would have burst had not the operative relief been provided rapidly. This duodenal distension is presumably of the nature of a neuromuscular ileus without actual mechanical obstruction. If diagnosed early, immediate operation would prevent the bursting of the duodenal stump.
